SGEM#50 looked at a RCT published in JAMA 2013 looking to see if a vasopressin, steroids, and epinephrine (VSE) protocol for IHCAs could improve survival with favorable neurologic outcome compared to epinephrine alone. I have not seen a validation study published.
These showed that with 22 incidents between 2011 and 2013, 43 people were harmed. Children who develop stridor may be treated with racemic epinephrine (0.25- 0.75ml of 2.25% racemic epinephrine to 2.5 According to a study completed in 2016, Nine states reported their chemical suicides for data points.
The first visit was back in 2013 for SGEM#50 : Under Pressure – Vasopressin, Steroids and Epinephrine in Cardiac Arrest. The Lancet 2020 This was an SGEM Journal Club episode recorded live at McGill University Grand Rounds. This was the third time coming to McGill University Department of Emergency Medicine to give Grand Rounds.
Most emergency drugs except for amiodarone and succinylcholine are based on ideal body weight [Emergency Medical Services for Children, Luten 2007] Epinephrine, dopamine, fentanyl, ketamine based on what child should weigh. PAWPER [Wells 2013, Silvagni 2022] Takes into account body habitus. Broselow does not.
The patient received 1 mg of epinephrine IV x2 with conversion of his rhythm to ventricular fibrillation (VF) for which he was defibrillated twice in the field. He requires low-dose epinephrine to maintain his mean arterial pressure (MAP) in the 60s mmHg and is transported to the cardiothoracic (CT) ICU. 2013 Jan 24;368(4):394].
When simple maneuvers fail, we proceed to topical agents such as lidocaine with epinephrine, oxymetazoline, anterior nasal packing, and electrical or chemical cauterization. 2013 Sep;31(9):1389-92. Epub 2013 Jul 30. Initial interventions include local pressure, ice, and forward head positioning for persistent bleeds.

1] The Adult Cardiac Arrest ACLS algorithm currently includes epinephrine and either amiodarone or lidocaine as recommended pharmacologic therapies. Time to epinephrine in out-of-hospital cardiac arrest: A retrospective analysis of intraosseous versus intravenous access. minutes versus 5.4 minutes). [5] Prehosp Emerg Care.
Fine ventricular fibrillation She received 2 mg epinephrine, 150 mg amiodarone and underwent chest compressions with the LUCAS device. Current Emergency and Hospital Medicine Reports (2013) 1:4352. She arrived in the ED 37 minutes after 911 was called, with continuing CPR. The patient was discharged neurologically intact.
With ventilations and epinephrine, she regained a pulse. A middle-age woman with h/o hypertension was found down by her husband. Medics found her apneic and pulseless, began CPR, and she was found to be in asystole. She was never seen to be in ventricular fibrillation and was never defibrillated.
Epinephrine should be considered as the vasopressor of choice in patients with decreased myocardial function. 2013 Nov;84(11):1500-4. Epub 2013 Aug 1. Heffner AC, Swords DS, Neale MN, Jones AE. Incidence and factors associated with cardiac arrest complicating emergency airway management. Resuscitation. 2013.07.022.
9 The only criterion for exclusion from this study was hypotension, defined as either the need for intravenous inotropic or pressor agents (dobutamine, dopamine, epinephrine, milrinone, norepinephrine, or phenylephrine) or a mean systemic arterial pressure below 60 mm Hg. N Engl J Med 2003; 348:1756-1763, 5/1/2013. Khot, MD; et al.
They continue CPR, get intravenous access, give a round of epinephrine and then wonder if they should start rapid cooling en-route to the hospital with some cold saline. Then came the Targeted Temperature Management (TTM) Trial ( Nielsen et al NEJM 2013 ). By-standard CPR is started and EMS is called. She is not in a shockable rhythm.
A 2013 study found that half of young Australians are dissatisfied with school-based sex education. Epinephrine was initially given to both groups after 5 minutes of chest compressions, this delay was based on a previous simulation study looking at length of time to site an UVC. Why does it matter? Sex ed” sucks.
Our allergy treatment toolkit includes an impressive suite of tools, and in the second episode of our two-part series on allergies, we focus on three in particular: antihistamine medications, allergy shots, and the epinephrine auto-injector (aka the EpiPen). “Epinephrine: a short history.” Then this is the episode for you.
He was defibrillated twice and received two doses of epinephrine, with return of spontaneous circulation. HRS/EHRA/APHRS expert consensus statement on the diagnosis and management of patients with inherited primary arrhythmia syndromes: document endorsed by HRS, EHRA, and APHRS in May 2013 and by ACCF, AHA, PACES, and AEPC in June 2013.
