Cleveland Clinic’s Geriatric Emergency Department (GED) model has significantly reduced hospital admissions and improved care for older emergency department (ED) patients. The GED model involves dedicated units, specialized practitioners, and a focus on early delirium detection, care transitions, and geriatric education. It operates in all 13 Cleveland Clinic EDs, with positive results: 17% admission rate for those seen by the geriatric team, compared to 55% for those not seen.

As hospital boarding, increased emergency department (ED) volumes, and complexity of patients have increased, so have wait times. Some physicians now coin themselves waiting room medicine specialists as departments schedule a physician in triage or attempt to evaluate patients in whatever spaces might be available. After years of training to fully undress a patient for an exam at ABEM General Hospital, patients may now routinely be treated in street clothes sitting in a hallway chair.
Each year, the number of active shooter incidents seems to climb. Hardly a week goes by without a broadcast reporting a gun-wielding assailant leaving behind a trail of victims. In 2023 alone, the Federal Bureau of Investigation designated 48 active shooter incidents. There were 50 in 2022.
