When I started as a paramedic in Hartford in January of 1995, I was given a 100-page protocol book to memorize. There were fewer than 50 protocols in the book, along with pages for 24 medications and 8 procedures. The book was approved by the two largest hospitals in Hartford. Paramedics didnt do 12-lead ECGs then.

Tranq dope (fentanyl-xylazine combination): A new horizon in opioid withdrawal treatment

Case A patient arrives via EMS from the bus station complaining of fever, vomiting, and back pain. The biggest change has been the gradual replacement of diacetylmorphine (heroin) by fentanyl and other synthetic opioids. Their back has worsened significantly over the past 24 hours with radiation down the left leg.

At one of the hospitals, while my partner finished up the run form in the passenger seat –shed teched the call–I announced I was going back into the ED to the EMS room to see if they had any pizza. They had more food in the EMS room, but I wasnt in the mood for Oreos or ice cream or browning bananas.
